How do I become
a member of Mountain State Blue Cross Blue Shield? |

If
you are employed, speak with the Human Resources representative
at your place of employment. The Human Resources representative
will tell you if your employer has a group policy with MSBCBS.
If there is a group policy, they can instruct you on the process
of enrollment. If the employer does not have a group policy, you
should contact the MSBCBS sales office to inquire about an individual
policy. The telephone listings are found under "Contact Us"
section of this web-site. |

I am already a
member of Mountain State Blue Cross Blue Shield. How do I request
a new and/or replacement identification card? |

You
may request the card directly from this website, by clicking under
"Member" and then "Request", select ID Cards
and complete the required information. The completed form will
be emailed back to Mountain State Blue Cross Blue Shield's Membership
Department to be updated. An identification card will be mailed
within 5-7 working days of receipt of request. |

How long does it take to receive
an identification card? |

Once the
request has been received, it will be processed immediately. The
card will be mailed to your home address within 5-7 working days. |
