Mountain State Blue Cross Blue Shield has added the new Explanation of Benefits and Remittance Advice documents to our Website. Our goal is to help our member and provider community better understand the new forms and be made aware of any future changes.

The document selection is located to the right of the web page. After the document has been selected other options are available at the top of each page. Document codes on some of the pages also provide information to additional links. These codes will appear as bright blue underscored text.
